Navigating a dementia diagnosis can be overwhelming—for individuals and their loved ones. The GUIDE Dementia Program at Stonehill Communities offers a compassionate, coordinated approach to dementia care that prioritizes quality of life, caregiver support, and the ability to remain safely at home.

Coordinated Care, When it’s Needed Most

This innovative healthcare model is designed to help people living with dementia—and their unpaid caregivers—access the support they need, when they need it. By reducing care fragmentation and offering personalized services, GUIDE helps delay or prevent the need for nursing home care.

What GUIDE Provides

The GUIDE Dementia Program provides value-based, person-centered care including:

  • Comprehensive assessment
  • Care planning
  • 24/7 phone support
  • Ongoing monitoring and support
  • Medication reconciliation and management
  • Care coordination
  • Respite care
  • Referrals and coordination of community-based services
  • Caregiver support

Who is Eligible

To participate in the GUIDE Dementia Program at Stonehill, individuals must:

  • Have a confirmed dementia diagnosis from a healthcare provider.
  • Live at home or in an independent or assisted living community in Iowa.
  • Be enrolled in traditional Medicare Parts A and B (not Medicare Advantage or hospice).

GUIDE services are fully covered by Medicare for those who qualify. Not eligible? Caregiver support is still available through our Caregiver Resource Center.
