At Stonehill Communities, we offer specialized memory care that promotes independence, ensures safety, and maximizes quality of life. Assisted Living Memory Care is available at Assisi Village. Long-Term Memory Care is available at the Stonehill Health Center. Both are located on the Stonehill campus on Dubuque’s historic North-End.

At Stonehill Communities’ Assisi Village, our Assisted Living Memory Care household is thoughtfully designed for individuals in the early to mid-stages of dementia or Alzheimer’s. The secure household features 14 comfortable bedroom-and-bathroom suites arranged around a spacious, inviting common area—where familiar faces and a sense of belonging greet residents each day.

Freshly prepared meals are eaten together in a restaurant-style setting, and a private patio provides a peaceful place to enjoy the outdoors and sunshine. Residents can attend Catholic Mass and prayer services in the Assisi Village Chapel and join in community celebrations and activities.

The Stonehill Communities Health Center serves those with more advanced care needs. Our Long-Term Memory Care household includes 16 individual bedroom/bathroom suites surrounding a welcoming living room, dining room, kitchen, and nurse station.

Meals are prepared and served in a restaurant-style setting, and a private rooftop patio provides opportunities to enjoy the fresh air and raised gardens. Residents can attend Catholic Mass, prayer services, and ecumenical services in the San Damiano Chapel and participate in activities and events throughout the Health Center.
The Stonehill Health Center has earned the from the Centers for Medicare and Medicaid Services (CMS) and received the Bronze – Commitment to Quality Award from the American Health Care Association and the National Center for Assisted Living. Plus 97 percent of residents and families would recommend Stonehill.

Stonehill offers compassionate support for individuals living with dementia at home as well as for the loved ones who care for them.
- Through the GUIDE Dementia Program, families receive case management, care coordination, and ongoing guidance—helping individuals remain safely at home, reduce hospital visits, and better manage the progression of the disease.
- Our Caregiver Resource Center provides no-cost education, resources, self-care opportunities, and meaningful connection for nonprofessional caregivers.
- In addition, Stonehill At Home offers in-home caregiver services and skilled nursing care, promoting independence, comfort, and overall well-being.
